Was ist ein DMP?

A DMP is a systematic treatment program for chronically ill patients based on the current state of medical knowledge (evidence-based medicine). The goal is to positively influence the course of the disease, avoid consequential damage, and secure your quality of life in the long term.

Your benefits of participation:

• Regular check-ups:We schedule fixed appointments (usually every 3 to 6 months) to closely monitor your health status.

• Prevention of Long-term Damage: Continuous monitoring allows us to identify complications early and take countermeasures.

• Targeted Training: You gain access to special educational programs to become an expert in your own health.

• Coordinated Care: We optimize the cooperation between your GP, specialists, and hospitals.

Good to know: Participation in a DMP is voluntary and free of charge for you. Many health insurance providers even reward participation with bonus points or by waiving co-payments.
